E911 Address Request Form and Instructions

- Addresses are NOT assigned to raw land, vacant lots, sewer systems, or electrical service boxes/posts.
- An address will ONLY be assigned AFTER a building permit has been issued by the appropriate Jurisdictional Zoning Authority:
Crow Wing County Planning and Zoning or Irondale Township or Crow Wing Township.
City residents must apply for an address through their City Hall.
- It is the applicants' responsibility to obtain permission from the appropriate agency to build a new driveway to the structure, BEFORE applying for an address.
If your driveway intersects one of the following: State Highway - contact MNDOT County Road - CWC Highway Department City Street - contact the City Clerk Township Road - contact the Township Clerk or Road Supervisor

Once a building permit has been issued and approval of the driveway location has been verified, the Address Request Form can be submitted with signature and payment. Please be aware that missing information could delay the address assignment.

It will take 2-3 weeks for the address to be issued, and the sign will be posted in 6-9 months by the Jurisdictional City, Township, or County authority. The new E-911 address is valid and usable after the applicant receives the notification letter in the mail. Copies of the letter are sent to the Auditor's Office to update the Public Records and the Jurisdictional Post Office. (The applicant will need to contact the Post Office before mail will be forwarded to the new address).

Irondale & Crow Wing Township residents must provide a copy of their building permit with the address request form.
