How do I Request a Certified Death Certificate?

You may request a certified death certificate from a local registrar at a county office or from the funeral home. If you request the certificate in person, you will be asked to complete and sign an application stating that you have tangible interest in the certificate. You will be asked to show identification.

To amend incorrect or missing information on a DEATH certificate, contact the Minnesota Department of Health at 612-676-5120.

Certificates may be applied for in person, but if the certificate is requested by mail, the application must be filled out AND notarized.

To apply for a death certificate by mail, please fill out the application and have it notarized, enclose check or money order for $13.00 for each certified copy of the death certificate ($6.00 for each additional copy of the same certificate), mail to our office, and we'll be happy to fulfill your request.
