Application must be made in person and the form must be signed in front of a passport agent at the time of application. (Children less than 18 years old must have parental signature(s), and if under age 16 must have signatures of BOTH parents and submit birth certificate even if prior passport is available.) ALL APPLICANTS MUST APPEAR IN PERSON, REGARDLESS OF AGE.
Processing - It takes about six weeks to receive your new passport. The time period can be shortened to 2 to 3 weeks by paying an additional fee to expedite your application, see fee schedule below. If a passport is needed more urgently, an in-person appointment can be made at one of ten passport agencies in the U.S.
2 Identical photos - 2X2" in size, full-face, must be printed on photo paper specifically for passport usage and must have a white background. Passport photos may be obtained from most photo studios and other businesses offering passport photo services.
Birth certificate - must be a certified copy from the county or state of birth and must be sent in with application - it will be returned with passport. (Hospital birth certificates or photocopies are NOT acceptable for passports.) OR Prior Passport - can be used even if expired (Under age of 16 MUST have birth certificate to prove parents' information) OR Naturalization Documents - must be original certified copy and must be sent with application (it is returned with your passport)
Picture Identification - current unexpired Driver's license, pilot's license, military identification, government employee card, state identification card, or notarized identifying witness form. If picture identification is other than a driver's license, additional identification documents are required, such as insurance cards, library cards, credit cards, hunting/fishing license, etc. and/or an indentifying witness with current picture identification.
Fees - Must have at least two check blanks, one made payable to the U.S. Dept. of State for each application and to the Crow Wing County Treasurer. (The U.S. Dept. of State accepts only checks or money orders, while the Treasurer's office can accept cash or checks)
Passport Books:
- Age 16 or older - total of $135.00.
$110.00 Check or Money Order payable to U.S. Dept. of State plus $25.00 to Crow Wing County Treasurer. Passport is valid for 10 years.
- Age 15 or younger - total of $105.00 -
$80.00 Check or Money Order payable to U.S. Dept. of State plus $25.00 to Crow Wing County Treasurer. Passport is valid for 5 years.
Passport Card
- Adults total of $55
$30.00 Check or Money Order payable to U.S. Dept. of State plus $25.00 to Crow Wing County Treasurer. Passport is valid for 10 years.
- Age 15 & under total of $40

Renewal Fees
- The cost to renew an adult's passport is $110.00; it is valid for 10 years.
To Expedite your passport To shorten waiting time to 2-3 weeks, add an additional $60.00 for each passport to the check payable to Passport Services: for example, the total cost of an adult's expedited passport is $170.00.
To further speed up the process, you may wish to purchase 1 pre-paid Overnight mailer from the U.S. Post Office and bring it along at the time you apply. Cost per mailer is currently $14.96.
Do you owe Child Support? Section 51.70 (a) (8) of Title 22 of the Code of Federal Regulations states in part that if you are certified to Passport Services by the U.S. Department of Health and Human Services (HHS) to be in arrears of child support payments in excess of $2,500 you are ineligible to receive a U.S. passport. If this applies to you, Passport Services strongly recommends contacting the appropriate State child support enforcement agency to make payment arrangements before applying for a passport.
